HOP Changing Lives

Client stated that they use to always call agencies for assistance but nothing ever seemed to work in her favor, until HOP. Community Health Worker (CHW) worked with member to encourage them to hold on and that something was going to happen in her favor. Provided information about HOP and how member could benefit from the services provided. One day CHW received a phone call and the member started expressing how they had secured housing for their family and how happy she was of the HOP program and the assistance they were able to receive.
