To be eligible for the Healthy Opportunities Pilot program an individual must have Medicaid AND be a Medicaid Managed Care Member. If you meet both of these criteria, please contact Member Services on the back of your insurance card. The Prepaid Health Plan will assess to see if you meet the necessary criteria listed below and inform you of the next steps. When contacting Member services inform them you are calling to speak with someone about receiving services through the Healthy Opportunities Pilot Program.
