Home Sweet Home

While working with client to create a stability plan to access their needs I learned that they were homeless and would oftentimes have to sleep in their car or at a friend’s house. Due to this, the client’s children were staying with another family member. During our meeting, I discussed with the client all the Services that HOPS provided and it was determined that the client needed help to get into sustainable housing.  The client disclosed that despite having a job, they were unable to provide the additional funds required for the security deposit, first month's rent, and utility deposits.  Client stated that they found out about the HOPS program from their care manager at Healthy Blue. The client was referred for rent/mortgage payment assistance, utility bill assistance, and move-in support.  Soon after, the client was able to locate housing. The client’s prospective home then passed inspection, and the client was able to move into her new residence with all their children. Client received move-in support items which consisted of beds, mattresses, and curtains for the home.  Client also received a Breathe Easy Kit through Healthy Home Goods. When I met with member to review their case, the client stated that were all doing well and enjoying their new home.  Client stated how grateful they were l for theprogram and thanked me for my assistance.
